ASSISTANCE PROGRAM
|
Provided through Guardian, the EAP provides
employees and their dependents 24-hour access to services related to
counseling, eldercare issues, parenting concerns, legal questions, financial
issues, and a variety of other topics.

DEATH & DISMEMBERMENT
|
Regular full-time employees are provided at
no cost AD&D insurance of $10,000. AD&D insurance is provided by
Guardian. New employees are eligible for AD&D insurance on the first day
of the month following 60 days from their date of hire.

LIFE
INSURANCE
|
Regular full-time employees are provided at
no cost group term life insurance of $10,000 the Guardian. Supplemental
voluntary life is also available to employees through Guardian at a cost of
$3.30 per $10,000 worth of insurance up to $50,000. New employees are
eligible for life insurance on the first day of the month following 60 days
from their date of hire.

Regular full-time employees are eligible to
purchase dependent life insurance $5,000 worth of coverage at a cost of $2.00
per month. New employees are eligible for dependent life insurance on the
first day of the month following 60 days from their date of hire.

Regular full-time employees are eligible to
participate in the flexible benefits plan. This plan allows employees to
deduct medical expenses, and day care expenses from their paychecks on a
pre-tax basis. New employees are eligible for flexible benefits on the first
day of the month following 60 days from their date of hire.

& PRESCRIPTION INSURANCE

Regular full-time employees are eligible
for group health insurance coverage through Blue Cross Blue Shield of
Oklahoma. New employees are eligible for medical insurance on the first day
of the month following 60 days from their date of hire. The PPO plan is paid
by both the City and the employee. Both the City and the employee pay a
portion of the dependent premiums. The following rates are per pay

RETIREE
MEDICAL INSURANCE
|
Regular full-time employee retiring from
the City of Altus have the option to continue medical insurance coverage
until the retiree becomes eligible for Medicare or is eligible to be covered
under another medical plan. Both the City and the retiree pay a portion of
the premiums. The following rates are monthly.

WORKERS’
COMPENSATION
|
All employees are eligible for workers’
compensation when injured in the scope of their employment for the City. An
employee will receive workers’ compensation payment during the recovery and
recuperation period provided by the City and at no expense to the employee.
An injured employee is entitled to medical aid that is required at the time
of the injury, and at any time thereafter, as may be necessary to cure and
relieve the effects of the injury.

RETIREMENT
PLAN (DB PLAN)
|
After 6 months Regular non-bargained for full-time
employees, are required to participate in OMRF (Oklahoma Municipal Retirement
Fund). The employee contributes 4% of their gross pay, which is tax deferred,
and the City contributes 12.97%. At the end of (10) years of participation in
OMRF, an employee is vested 100% in the City’s contributions. Employees are
eligible to retire at age 65 with 10 or more years of vesting service or age
62 with 25 or more years of vesting service. For more information contact

DEFINED
CONTRIBUTION RETIREMENT PLAN (DC PLAN)
|
After 6 months Regular non-bargained for
full-time employees, are eligible to participate in OMRF (Oklahoma Municipal
Retirement Fund) defined contribution plan. The employee contributes after
tax dollars up to $49,000 including employer contributions, and the City
contributes 1.49%. At the end of (10) years of participation in OMRF, an
employee is vested 100% in the City’s contributions. Before vesting return of
employee contribution account balance (in any) is allowed. Employee may
borrow the lesser of $50,000 or 50% of vested account balance with a minimum

DEFERRED
COMPENSATION
|
Regular full-time employees can save
additional money for retirement through a 457 deferred compensation plan.
Employees can choose to voluntarily defer monies through The Hartford
Insurance Company. For more information contact Human Resources.

VACATION
LEAVE
|
Eligibility Regular full-time
and part-time employees are eligible to accrue annual leave. Regular
full-time employees are not eligible to take annual leave until they have
been employed for 12 months. Each month eligible employees are credited
with 2 hours of attendance leave. Upon completion of 6 months of employment
the employee may use the attendance leave. Unused leave balances will carry
over to the next year up to a maximum accrual of 120 hours (168 for fire
shift personnel). Employees may be paid for unused vacation balance upon
termination after 12 months of employment.

alternate accrual schedules please consult collective bargaining agreements
for details.

|
All regular full-time employees shall
accrue one full working day for each calendar month of service.

LEAVE
WITHOUT PAY
|
Leave without pay is an absence from work
in a non-pay status and is granted as a matter of administrative discretion
or as it applies to the Family Medical Leave Act. Leave without pay is only
available one all appropriate paid leave has been used. (Some restrictions
apply).

COMPENSATORY
LEAVE
|
Non-exempt employees may elect to accrue
compensatory time (up to 40 hours) off in lieu of overtime pay. Supervisor
approval is required. Compensatory time is accrued at a rate of one and a
half (1 ½) hours comp time for each hour of overtime worked.

HOLIDAYS
|
Regular full-time employees are granted 12
paid holidays per year. Part-time employees who work more than 25 hrs per
week will receive holiday pay at a prorated amount.
